Quality Improvement (QI) Initiative for Alzheimer's Disease Care

Background

Lilly USA, LLC, a leading pharmaceutical company, is seeking proposals for a Quality Improvement (QI) initiative aimed at enhancing the care of patients living with early symptomatic Alzheimer's disease (AD), specifically targeting underserved and underrepresented groups. This initiative focuses on identifying and addressing healthcare gaps, standardizing processes, and improving patient outcomes through evidence-based practices.

Successful applicants will receive funding from a total budget of approximately 500,000 to 1,000,000, with individual grants varying based on the proposal's evaluation and associated costs.

Focus Areas:

  • Increasing the number of people from underserved and underrepresented groups who receive an accurate and timely diagnosis of AD
  • Enhancing the provision of guideline-based care and treatment for patients with AD
  • Addressing systemic barriers to equitable care throughout the diagnostic and treatment pathway

Eligibility:

  • Large integrated health delivery systems, ACOs, hospital systems, insurers, and other entities capable of measuring and implementing interventions to address healthcare gaps
  • Applicants with a vested interest in improving AD care and the ability to demonstrate sustainability and scalability of successful interventions

Proposal Requirements:

  • Detailed description of health system practice gaps and objective data sources used to measure gaps in processes, patient care, and outcomes
  • Clear intervention plans with estimated improvements and patient impact
  • Objective outcomes measures for process, clinician performance, and patient outcomes
  • Qualifications and experience of the project leader and collaborators
  • Communication, sustainability, and scalability plans

Submission Instructions:

  • Include "QI RFP: [title of program]" in your proposal submission
  • Limit proposal length to 30 pages (excluding references and budget)
